1. Standard Questions
The standard questions are mandatory questions set by Football Australia as part of the signup process. Organisations are limited to Seven questions per level and cannot exceed that limit.
2. Governing Body Questions (Football Australia, Member Federations and Associations)
These can only be edited or changed by the respective organisation.
3. If you do no want to add any additional questions, select 'No'
4. If you do want to add additional questions, select 'Yes'
5. This will default to the 'Add Link' option
'Add Link' allows you to add the URL to your additional questions document.
6. To change to the 'Input Questions' type, select the drop down option on the 'Add Link' tab
7. You change the question format by selecting the drop down option on 'Question Type'
Short Text: Participant's can write a response to the question.
Selections: Participant's can choose between a range of options.
Selection (Yes/No): Participant must select Yes/No to the question.
External Link: Participant's can access the URL to an external form.
8. Mark as 'Required' if you want to make the question mandatory for all participant's
9. Select 'Next' and review your product details
7. Additional Questions
All additional questions added by each level can be reviewed. Only the respective levels can alter their questions
8. Click on Submit
9. Submitted
Only successful creations will have this page displayed to the administrator
